5. ID NUMBER:
Provide a valid ID number in this field. Any characters entered here will be replaced by the actual ID number. For example, if you enter 12345678 in this field and click Apply, the system will replace it with 12345678 when saving your application form.
6. DATE OF BIRTH (YY/MM/DD):
The Date of Birth is the most important information in this application form. All applications must be submitted by citizens born before or on 12/02/2012.
The format for DOB is YY/MM/DD (for example: 08/23/1961). Please do not enter your birth year as part of the date, only enter the month and day.
7. GENDER (F/M):
Please choose the gender category that best fits you:

8. HOME ADDRESS (STREET NAME & NUMBER / P.O. BOX / RURAL AREA):
For example, if your home address is 123 Main Street, you would write: “123 Main Street”. If it is a rural area and there is no street number or box number, write “Rural Area”.
9. CITY/TOWN:
- City/town: This is the place where you live. It can be a town or city in South Africa, but also an international place. For example, if you were born in London and now live in Cape Town, then you would write “London” as your city/town. If you were born in Johannesburg but moved to Durban and now live there, then you would write “Johannesburg” as your city/town.
If your business is based on more than one location (for example, if it has offices in two different towns), please enter all locations here (each on a separate line)
10. PROVINCE CODE (01-09):
The province code is a two-letter designation that post offices use to deliver mail to the correct province. The province code for Newfoundland and Labrador is 01, while Alberta is 04 and British Columbia is 03.
You can find your province’s code by visiting Canada Post’s website or calling them at 1-888-550-6333.
11. POSTAL CODE:
The postal code is a unique code used by the South African Post Office to deliver mail and for other purposes.
Postal codes in South Africa are 5 digits long. The first two digits represent the area post office, the third digit represents a delivery route (within an area post office), and the last two digits represent individual addresses within that delivery route.
Postal codes are not unique to each suburb or town, but they are unique across all of South Africa’s metropolitan areas (including capitals like Johannesburg and Pretoria).

- Country of Birth: The country where you were born and raised before moving to another country.
- Country of Citizenship: The country where you hold a passport, if applicable.
